LPN Home Health

As a Licensed Practical Nurse, you will be responsible for coordinating nursing care for patients based on established clinical nursing practice standards. This will involve delivering patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation.

The Licensed Practical Nurse is responsible for assisting with the coordination and supervision of quality nursing care services for assigned residents that reflect the facility’s philosophy, follows established standards for Home Health and/or Residential nursing services, and promotes the individual independence of residents served.

• Has an active, in good standing, Indiana license as a Licensed Practical Nurse and graduated from an accredited school of nursing.
• Has strong organizational and time management skills.
• One-year professional nursing experience providing direct care to residents in long-term care, acute care, restorative care, or a geriatric setting.
