LifeSpan Companion Care puts a face on the idea of aging in place. It’s the face of caregivers who ensure seniors’ safety and independence wherever they call home. This could be their private home, independent or assisted living, a memory care campus or a long-term care community. “Companion” is a fitting name because we’re not doctors — we’re your friends. We’re around when you need us — on your terms. Our caregivers provide non-medical personal services, supporting seniors with dignity and giving their loved ones respite and peace of mind.

1 . Follow the instructions of the Agency Manager in providing care.
2. Provide assistance with hygiene such as bathing, oral care, and dressing.
3. Perform and record accurate measurements (e.g., intake/output as instructed in the service plans).
4. Observe and report any safety hazards found in the client’s home or any significant observations regarding the client.
5. Report patient complaints to the office staff
6. Maintain patient confidentiality and adhere to the agency Policy and Procedure manual.
7. Possess the ability to follow written and oral instructions.
8. Report abnormal findings in client’s conditions as observed and per the client’s service plan.
9. Perform the following task when instructed by the Agency Manager:

  • Documentation of environmental situations and client behaviors
  • Ambulation, positioning, and turning
  • Transportation
  • Personal hygiene and elimination
  • Meal Preparation
  • Socialization
  • Self-Care
  • Teaching reinforcement that has been planned by the family and Agency Manager
  1. Graduate of an accredited High School or equivalent preferred.
  2. Ability to read, write and follow directions.
  3. Must meet agency personal care attendant training requirements.
  4. Work positively and favorably with patients, families, and staff.
  5. Demonstrate compassion, responsibility, and a cheerful attitude.
  6. No findings on the nurse aide registry or Medicare exclusion list relating to client abuse, neglect, or misappropriation of client property.
